[PATCH] pi-futex: robust-futex exit crash fix

Fix pi_state->list handling bugs: list handling mishap, locking error.
Plus add more debug checks and fix a few style issues i noticed while
debugging this.

(reported by Ulrich Drepper and Jakub Jelinek.)
